Please read this form carefully and be aware that in signing up and participating, you will be agreeing to the terms of this Release and Waiver Form for any and all claims for injuries arising out of your participation in classes and activities at Body Art Noire. As a participant, I acknowledge that I am participating in a physical conditioning program such as yoga and use of the facilities at Body Art Noire at my own risk. I assume the risk connected with the participation in the physical conditioning program, represent that I am in good health, and suffer from no physical impairment which would limit my use of Body Art Noire facilities. I acknowledge that Body Art Noire has not and will not render medical services including medical diagnosis of any claim, demand, cause of action of any kind whatsoever for an account of death, personal injury, property damage or loss of any kind resulting from or related to my use of the facilities or participation in any sport, exercise, yoga or activity within or without the studio premises and I agree to hold Body Art Noire, its owners, affiliates, agents or employees or any other person or company for Body Art Noire harmless from the same. I understand that the instructor may offer adjustments as part of teaching and coaching. Adjustments mean appropriate physical contact and showing proper form, movement and/or placement. I am fully responsible for telling the instructor if I prefer that he/she not include adjustments. I understand the nature of the program for which I am registering, and have read and fully understand the Waiver, Release and Hold Harmless Agreement. I further understand that any advice or warnings of the particular risks of this program that I subsequently receive will be incorporated by reference into and become a part of this Agreement. If the Participant is under 18 years of age, as Parent/Guardian, I consent to the about terms and conditions.
